This Privacy Policy explains how Patient Driven Care, Inc. ("us," "we," or "our"), a Texas corporation, collects, uses, and protects your information within the MyPatientAssistant.com application (the "Application").
By accessing or using the Application, you agree to the terms of this Privacy Policy. If you do not agree with any part of this policy, you do not have permission to use the Application.

- b.well Connected Health, Inc.: Our trusted health data intermediary used to securely connect and synchronize your health information across your various patient portals.
- Core Cloud Infrastructure: Secure cloud servers and database utilities used strictly to host, protect, and deliver our website and application to you.
- Internal AI Models: Secure, specialized large language models utilized strictly within our platform to power application features and optimize performance.
Sale of Your Information and Secondary Data Uses
We do not sell your personal information, nor do we sell, license, or monetize your health, claims, or exercise data.
Restrictions on Secondary Uses and De-identified Data
We do not sell, license, or monetize your personal information or health data. We strictly limit your data usage to operating and providing the Application.
- No Marketing or Advertising: We never use or share your health profile, diagnoses, or claims history for commercial marketing, targeted advertising, or consumer profiling.
- No Anonymized Data Monetization: We do not compile or license anonymized, aggregated, or de-identified datasets for third-party commercial exploitation or research.
Any future downstream or secondary use of your data will strictly require your separate, explicit, and revocable opt-in consent. You can manage or withdraw your consent at any time by emailing contact@patientdrivencare.com.
Data Retention
We only keep your personal and health information for as long as your account is active, or as long as necessary to provide you with the Application's features. If you choose to delete your account, we will permanently purge or de-identify your data from our active databases within 30 days, unless we are legally required to retain certain billing or security records to comply with financial or fraud-prevention laws.
Cookies
The Application only uses essential cookies strictly required for security and core functionality, specifically to manage your secure login session via Auth0. We do not use cookies for tracking, profiling, or analytics.
Children's Privacy
The Application is not intended for or directed to individuals under the age of 18. If we learn that a child under 18 has provided us with personal information, we will use that data only to notify them that they cannot use the Application, and we will promptly delete it.
Your Rights
Depending on your jurisdiction, you have the following rights regarding the personal and health information we process within the Application:

- Verifying Your Identity: To protect your privacy, we have to make sure it's actually you asking. Please include your name, email address, and account name with your email.
- Having Someone Else Ask for You: You can authorize an "agent" to make a request for you. For security, they must provide your written, signed permission or a valid power of attorney before we can release any data.
What to Expect (Timelines and Appeals)
- How Long It Takes: We will handle your request within 45 days. If we need more time for a complex request, we will let you know in writing (up to 90 days total). If a legal exception prevents us from fulfilling your request, we will explain why.
- Your Right to Appeal: If we deny your request and you disagree, you can email us to appeal our decision. We will review it and give you a final response within 60 days.
- Cost: Exercising your privacy rights is completely free. We would only ever charge an administrative fee if someone abuses the system with excessive or repeated spam requests.
Third-party Websites
The Application may contain links to external websites or portals (such as insurance providers or health systems). We do not control these third-party platforms and are not responsible for their content, security, or privacy practices. Once you leave our Application, this Privacy Policy no longer applies, and we encourage you to read the privacy policies of those external sites.
Do Not Track
Our Application does not currently respond to browser-based "Do Not Track" (DNT) signals because there is no uniform industry standard for how websites should interpret them.
Changes to Privacy Policy
We may update this Privacy Policy from time to time. When we do, we will post the revised version on this page and update the "Last Updated" date at the top. If we make any major changes to how we handle your personal or health information, we will notify you directly through the Application or via email before the changes take effect.
